Number of full-time, year-round employees:

• 1 - 2      $389
• 3 - 5      $461
• 6 - 10    $564
• 11 - 15  $760

If you are a restaurant or lodging establishment, nonprofit, or have more than 15 full time employees the above rates do not apply.
